Bennett fracture orthobullets11/21/2023 The ‘mirrored’ Bennett fracture of the base of the fifth metacarpal. Fracture-subluxation of base of fifth metacarpal. The causative mechanism is axial overload along the first metacarpal with simultaneous flexion. Management of Intra-Articular Metacarpal Base Fractures of the Second Through Fifth Metacarpals. Principles This injury, described by Bennett in 1882, is a fracture subluxation of the first carpo-metacarpal joint, with a sizeable palmar anterior marginal fracture fragment. The surgical treatment of fractures of the base of the fifth metacarpal bone. The fracture-dislocation was reduced with local pain relief and reimaged before the patient was taken to the operating theatre for K-wiring under image intensifier control. A clinical and radiographical study of 64 cases. The lateral hand image demonstrates the angular displacement of the 5th metacarpal base. There is also a small bony fragment demonstrated at the base of the 5th metacarpal adjacent to the cortex of the hamate ? donor site fracture, Orthobullets & oral boards, 2014 (level of evidence 5) Thomas B. The oblique hand image demonstrates dorsal displacement of the dislocated 5th metacarpal. Bennett fracture is the most common fracture involving the base of the thumb. The PA hand image demonstrates a fracture dislocation of the base of the 5th metacarpal with ulnar displacement. The patient's right metacarpophalangeal joint demonstrated an abnormal lack of prominence. His right hand was very painful and he had an abnormal prominence dorsally along the ulnar aspect in the region of the base of his 5th metacarpal (arrowed). This patient presented to the Emergency Department following a fight. extra-articular fractures through the first metacarpal base Bennett. In his article, three of the twelve patients had a Rolando fracture 3. Novelline, The Radiology of Emergency Medicine, 3rd ed, Williams and Wilkins, 1993, p452 - 455). It is named after Silvio Rolando (Italian surgeon) who first described the fracture in a small case series of thumb metacarpal fractures in 1910. In further analogy with the Bennett Fracture, the large distal fragment of the base of the fifth metacarpal is proximally retracted by the flexor and extensor carpi ulnaris muscles, and open reduction and internal fixation are recommended to reduce and stabilise the fracture fragments. The form and severity of this fracture depends on the position of the hand at the moment of hitting the ground. Bennett fracture: defined as an intra-articular 2-part fracture of the base of 1st metacarpal (see also Reverse Bennett fracture below) Rolando fracture: similar to Bennett fracture, but the prognosis is worse. "Because the fracture-dislocation of the base of the fifth metacarpal is pathologically and radiographically similar to the Bennett fracture of the thumb metacarpal, it is referred to as a " reversed Bennett", " ulnar Bennett", or " Tenneb" (Bennett spelled backwards) fracture". The Reverse Bennett fracture-dislocation is caused by a falling or any types of trauma affecting the patients hand and specifically the 5th metacarpal joint.
